How to Submit and Track ADA Reservation Requests

All ADA reservation requests must be submitted through the ADA Reservation Requests Microsoft Form in Teams. Do not submit requests through the Posts section instead of using the form.

This process standardizes the information collected and keeps all ADA reservation requests in one centralized location.

1. Find the ADA Reservation Requests Form

  1. Open Microsoft Teams.
  2. Navigate to C&J Employee Hub.
  3. Open the CJ ADA Accommodations channel.
  4. Select the ADA Reservation Requests form along the top of the channel.

2. Complete and Submit the Form

Enter the required customer and travel information. The form collects:

  1. Customer name
  2. Customer phone number
  3. Date of travel
  4. Bus departure time
  5. Departing location
  6. Arriving location
  7. Logan Airport terminal, if applicable
  8. Level of accessibility needed, such as FSR, WC Lift, XFR, or walker
  9. Whether seats need to be removed
  10. Order number
  11. Additional notes

Provide accurate and complete information so the team can coordinate the customer's accessibility needs.

3. Where to Find Submitted Requests

After a form is submitted, the information automatically populates both the ADA Tracker and the Posts tabs. A notification is also automatically posted for team visibility.

  • ADA Tracker: The primary record for ADA reservation requests. It provides a sortable, running list that employees can use to view, track, and reference submissions.
  • Posts: Displays submitted responses and provides visibility to the team.

Use the ADA Tracker as the main reference for reviewing and tracking requests.

4. Driver Run Sheet Updates

Jack periodically reviews submitted requests and records the appropriate information on the driver run sheets.

Submitting the form ensures the request is captured in the tracking process. The team should use the established workflow to ensure accessibility needs are communicated appropriately.

5. Important: Do Not Use the Calendar for Tracking

The calendar component was removed from the workflow because submitted forms did not automatically populate it. Reservations had to be added manually, and the calendar only displayed one week at a time.

The ADA Tracker is the primary record for all ADA reservation requests. Do not rely on the calendar to track these requests.

6. Review and Feedback

Employees are encouraged to review the form and ADA Tracker and share feedback about any missing fields or potential workflow improvements.
